More and more young people with life-limiting or life-threatening conditions are living into adulthood, and therefore making the move from a children’s hospice to adult services.
At Richard House we aim to help make this transition as smooth as possible, through the transition programme that we run with St Joseph’s Hospice, an adults’ hospice in Hackney.
The transition programme
Together we support children and young people from the age of 14 as they grow into adulthood and develop life skills around issues like self-esteem, relationships, sexuality, drugs and alcohol, bullying and health and wellbeing.
We also work closely with other professionals in health care, social care and education to discuss each young person’s specific health and social needs and ensure all care is coordinated. We also facilitate support groups for young adults.
