At Richard House we aim to help make this transition as smooth as possible, through the transition programme that we run with St Joseph’s Hospice, an adults’ hospice in Hackney.
The transition programme
Together we support children and young people from the age of 14 as they grow into adulthood and develop life skills around issues like self-esteem, relationships, sexuality, drugs and alcohol, bullying and health and wellbeing.
We also work closely with other professionals in health care, social care and education to discuss each young person’s specific health and social needs and ensure all care is coordinated. We also facilitate support groups for young adults.
“What makes Richard House really special is that it moves along with you. If I could sum it up in one word it would be freedom. You don’t feel shackled down by your impairments or the things that you have to deal with.”
