If your child has a life-limiting or life-threatening condition, or if they have complex health needs, we are here to support you to take that step between hospital and home

Babies with complex care needs, children and young people with life-limiting conditions or who have complex health needs can often be in hospital for long periods of time. 

Our step-down care service is for these children who have spent a very long time in hospital and whose parents and carers need to be prepared for their safe discharge home in a home form home environment.

Our care team is on hand to help and advise as required, and train and support parents and carers on how to give their children medication and use medical equipment so that their child can be cared for at home instead of hospital.

This training improves the confidence for parents and it allows children to go home from hospital sooner and be a family.
